Q: How do I change my address?
A: If you are a registered user of our OnLine Service Center (you
have established a user name and password) you can update your address
by clicking on "Address Change" on the left menu in the OnLine Service
Center. If you are not a registered user, you can download and complete the
Change Address Form. Or, you can
contact
Customer Service and provide your new address in your message.

Q: What does my policy cover?
A: Please refer to your Certificate of Insurance or Policy.
The Schedule of Benefits page gives highlights and policy benefit
maximums. The complete policy provides descriptions and specifications
for each policy benefit.
If you are unable to locate your Certificate of Insurance or Policy,
you may download a Request For Duplicate Policy form, which
includes the mailing address and fax number for the Customer Service
Department. You may also request a duplicate policy by calling the Customer
Service Department at 1-800-654-8489 from 8:00am to 4:45pm CST.
